Bio-identical hormonal replacement (BHRT)
Removal of ovaries before menopause may increase the risk of dementia in old age. This is according to an article from Neurology 2007; 69(September11): 1074-1083. This could be another reason for women in this situation (hysterectomy that includes the removal of the ovaries) and women who are entering and/or completed menopause (all situations cause low estrogen, progesterone, testosterone and other hormones) to consider bio-identical hormonal replacement.
Standard Hormonal Therapy (HRT)
Several years ago when hormonal replacement began most commonly just estrogen was used. This unfortunately was found to cause an unacceptably high incidence of uterine cancer.
Since mother nature cycles estogen and progesterone (normally in the menstral cycle the first 2 weeks is estrogen and the second 2 weeks is progesterone predominant). Progesterone was then recommended to be used in a cycling fashion. Fortunately this decreased the incidence of uterine cancer.
The lession to me is mother nature (the body) for the most part knows best. When treating a patient one should try to be as natural as possible. Alway try to mimic what the body does when its healthy.
Woman’s Health Initiative Study in 2002 found that women taking standard estrogen derived from pregnant horse urine and synthetic progesterone had increased heart disease, cancer, and strokes. Because of this study standard hormonal replacement (HRT) is rarely utilized. This has left thousands of women without the several possible benefits of HRT and some to suffer from menopause.

Conjugated equine (horse) estrogens plus medroxyprogesterone acetate (synthetic progesterone) hormonal replacement and Breast Cancer Detection:
It was found that conjugated equine (horse) estrogens plus medroxyprogesterone acetate (synthetic progesterone) had a negative breast cancer detection and outcome result.
The results indicated that patients on these medicines had increased mammogram abnormalities, less sensitivity for cancer detection and increased breast biopsies. Breast cancers were significantly increased and were diagnosed at higher stages. This according to an article in the Archives of Internal Medicine Feb. 25, 2008
Standard Hormonal Therapy (HRT) / Where Did It Go Wrong?
Source: estrogen was derived from the urine of pregnant horses and included unnecessary forms of estrogen found only in horses. The progesterone utilized was a synthetic compound.
Both estrogen and progesterone were not chemically identical to that of the human body.
HRT was used to reduce postmenopausal symptoms and not to correct hormonal imbalances.
Blood levels of estrogen, progesterone, DHEA and testosterone in the majority of patients were not taken prior to or during treatment.
Estrogen subsets were not measured. Estrone when elevated can cause cancer. Estrone can be lowered with natural products.
Typically one dosage was prescribed for all women regardless of their body weight and/or baseline blood levels. Thus, a 98-lb woman would receive the same amount as a 250-lb woman (one pill fits all).
Therefore, standard hormonal replacement was not based on the individual’s true physiologic requirements (medicine for the masses).
Bio-identical Hormones (BHRT)
Bio-identical hormone replacement therapy [synthetic or nonsynthetic hormones (derived from plants) that are identical in chemical structure to hormones produced naturally] has many possible advantages such as:
Other hormones ( DHEA, testosterone, IGF-1, estrone, cortisol, pregnenolone, etc.) are balanced as naturally as possible
Fewer side effects
Decreased cancer
Improved cardiovascular and bone health
Consider by most Anti-Aging, Complementary, Holistic medicine authorities to be anti-aging
Decreased menopausal symptoms
Increased libido, vaginal lubrication, and
breast tone
Decreased wrinkles, hair and memory loss
Weight loss with decreased fat tissue and increased muscle mass
Increased health, lifespan and sense of well being
Not all patients are candidates for either standard and bio-identical hormonal replacement.
They should be used under close supervision of a qualified and expierenced physican.
The patient should have regular blood tests, physicals ( including pap smears), breast imaging, pelvic ultrasound & self examinations.
Patients should also maintain a healthy life style including but is not limited to the following:
Positive attitude
Healthy balanced diet leaning towards vegetarianism and utilizing high density foods (low calorie, high protein, vitamin and mineral rich foods)
Drinking eight to ten glasses of pure water
Regular exercise (aerobic, anaerobic, mind)
Adequate sleep, rest, and stress control
Healthy relationships
Regular medical (traditional) check ups, blood, diagnostic testing and treatment in a balanced approach with alternative therapies
Stress management
Spirituality
Current Medical News
The “American Medical News’’ ( Volume 51,No. 7, February 18, 2008) published by the American Medical Association stated “Data analysis suggests the treatment (hormonal replacement) is safer in the decade after menopause than later on. Also, the FDA acts to rein bioidentical hormones.
Low testosterone / Wall Street Journal Feb. 26, 08
Discussed low testosterone in males. Entitled: Grumpy Old Men: Maybe It's Your Testosterone
The St. Louis University Androgen Deficiency in Aging Men questionaire asks ten questions to determine if you might have low testosterone. This includes:
1. Decreased sex drive 6. Sad and/or grumpy mood
2. Weaker erections 7. Lack of energy
3. Decreased strength or indurance 8. Difficulty staying awake after dinner
4. Decline in height 9. Recent deterioration in ability to play sports
5. Decreased enjoyment of life 10. Recent deterioration in work performance
You might have low testosterone if you have either of the first two and at least three of the other symptoms discussed above.
IT SHOULD BE NOTED THAT THERE ARE MANY REASONS FOR LOW TESTOSTERONE.
The cause should be found and corrected. It is an over simplification to assume that low testosterone is all ways due to decreased testicular function.
There are many causes and different appropriate treatments.
One should seek a physican that has experiece treating low testosterone.
You might not need to injections, creams or jells of testosterone.
I have found several way of making ones own body make testosterone naturally.
This syndrome has also been called male menapause.
This was discussed in the Wall Street Journal Feb. 26, 08.
Please see my web site and blog entitled male menapause (to be created by 4/08).
